A community health assessment lays a factual foundation for community health decision making. Recognizing this, Sky Lakes Medical Center, Klamath County Public Health, Cascade Health Alliance, and Klamath Open Door Family Practice came together in collaboration with various community partners to better understand the health needs in our community.

 

This CHA formally assesses and documents the health of our community utilizing a coordinated and collaborative process. It is the first step towards the creation of a healthy, vibrant, and prosperous Klamath.

 

The primary goal of our CHA is to better understand the health of our community and to develop local strategies to address our community's specific needs and identified priority issues. The CHA will be utilized to set community health priorities, establish benchmarks and monitor trends in the health status of Klamath County residents and develop a platform for collaboration to improve health status.
