The following web maps show the analysis of 60,000 de-identified (as per HIPPA requirements) patient records obtained from the Sky Lakes Medical Center. These data provided patient conditions for stroke, heart disease, diabetes, obesity, high blood pressure, smoking, high cholesterol, and total cost of care. These data were aggregated to census block group areas so as to depersonalize the data. As such the characterizations are deemed to be representative of the census block group and not of any individual household.
