Outpatient Care Management Department offers the Diabetes Self-Management Program. Cascade Health Alliance Employs four certified diabetes educators who offer one-on-one pre-diabetes education and fully covers diabetes supplies for their members. Klamath Open Door Offers one-on-one diabetes and pre-diabetes counseling for patients. Klamath Open Door plans to offer Diabetes Self-Management Programs in the future.
